If the existing insulation is reasonably level and evenly distributed and has not been nested in by vermin you could install additional insulation over the top to increase the thermal efficiency to current standards, this application also seals in the dust that has settled on the insulation over the years. Removal of Insulation
If you have the older type rockwool, fibreglass or loose cellulose insulation my recommendation would be to remove this insulation then vacuum the ceiling clean of any dust and extraneous matter, then re-installing a more user friendly insulation eg Earthwool, Pinkbatts or Polyester Batts.
The build up of dust over a period of years can filter into the home through ceiling openings e.g. down lights, exhaust fans and vents. This could be harmful to your health especially those with Asthma and respiratory conditions.
Under floor insulation

Polyester Under floor insulation is designed to provide thermal insulation under floors of new and existing timber framed buildings.
